How do I register for healthcare services at an HCS clinic?
At the beginning of each school year, students receive consent forms for their parent or guardian to complete and sign. Students need to return completed consent forms or packets to school so HCS staff are able to treat students during the school year. Any specific service, such as flu vaccinations, require another consent form that is sent home for parents or guardians to complete and return to the school before students can receive the service or vaccination.
What should students bring to their first clinic visit?
Students should bring the consent forms that are completed and signed by their parents or guardians to their first visit at a health center to be able to be seen by anyone at the school's health clinic.

Click here to download the consent packet for students to be seen at a school-based health center (or health clinic). Complete the required pages, sign them, then return the completed forms to your child's school.
School nurse services are provided to varying degrees at each school that does not have a health center. Any specific programs or services may require a consent form that will be sent home with your student.

Your child's school will provide you with the specific emergency forms required for East Baton Rouge Parish School System.
